  • If you prefer to pay by check, download and mail us your application, order form, and payment to the address listed below. Most supply orders placed online before noon Central Time (except for weekends and holidays) will be shipped by UPS the next business day. Please allow 2-5 days for shipping (view shipping information).

  • Use a work street address for faster delivery service by UPS. UPS tracking information for residential shipments is not guaranteed.
